Intake Materials
To prepare for the consultation, please print the five documents listed below. Please complete the intake packet and bring it with you to the consultation session, and also sign and bring with you the Evaluation and Treatment Agreement and the Privacy Policy consent. The page of directions will help you locate the center, aid you with parking, and provide the code number you'll need to get into the suite.

After your initial consultation by phone, Ms. Courry will email you an invitation to log into the secure portal system she uses for paperwork. All forms can be accessed through the portal. Some forms can be completed online and some will need to be printed and brought with you to your session.
